ABSTRACT
INTRODUCTION:
Nearly half of the upper gastrointestinal cancers worldwide occur in China. In China, screening coverage shows a 'higher in the north, lower in the south' pattern. As the only national prospective cohort site for esophageal cancer in South China, the epidemiological characteristics of the disease in the Chaoshan region remain unclear. The purpose of this study isTo analyze the risk factors for upper gastrointestinal cancers and precancerous lesions in the Chaoshan region, providing a basis for targeted regional prevention and control.

METHODS:
From March 2019 to October 2025, a retrospective case-control study was conducted on high-risk individuals aged 40–69 in Chaoshan. The target sample collected valid data from 2,273 cases. Logistic regression was used to analyze 17 independent variables.

RESULTS:
The prevalence of upper gastrointestinal cancer and precancerous lesions was 16.41% (373/2,273). Univariate analysis showed that 10 factors were associated with disease risk (P<0.05). Multivariate analysis indicated that being male (OR=1.608, 95% CI: 1.169–2.211), age groups 45–69 years (reference 40–44 years, OR=2.055–2.799), alcohol consumption (OR=1.511, 95% CI: 1.189–1.920), and households with ≥8 members (OR=1.970, 95% CI: 1.103–3.519) were independent risk factors; households with 4–7 members (OR=0.749, 95% CI: 0.569–0.984) and consumption of meat, eggs, dairy, and legumes (OR=0.096–0.585) were independent protective factors (all P<0.05).

CONCLUSIONS:
The detection rate of upper gastrointestinal cancers and precancerous lesions in the Chaoshan region is relatively high. Screening efforts should focus on high-risk groups such as men, the elderly, alcohol consumers, and large families. It is recommended to advocate for a balanced intake of meat, eggs, dairy, and legumes, and to develop differentiated prevention and control strategies based on age and family structure, in order to improve regional prevention and control effectiveness.
