This study aimed to investigate the association between current severe asthma symptoms and households fuel use among preschool children in Mabopane, South Africa. This was a cross-sectional study conducted in the City of Tshwane Metropolitan Municipality in Gauteng, South Africa. A total of 1840 parents and guardians of preschool children completed the modified International Study of Asthma and Allergies in Childhood questionnaire. Data were analysed using multiple logistic regression analysis. The prevalence of current severe asthma symptoms was 15.4%. The use of gas was significantly associated with severe asthma symptoms (OR =3.20; 95% Cl: 2.08- 4.91). The use of open fire (paraffin/wood/coal) was positively associated with current severe asthma symptoms and (OR= 1.87; 95% 0.98- 3.55). The study shows a significant association between current severe asthma symptoms and household fuel use among preschool children in Mabopane, South Africa.
