Barriers for adult vaccination: results of the qualitative survey with adults and healthcare workers in three regions of the Russian Federation

ABSTRACT
BACKGROUND:
Unlike childhood vaccination, where organizational and methodological approaches are well-developed, less attention is paid to adult vaccination. Objective of the survey was to study the factors that hinder vaccination of the adult population in three regions of the Russian Federation.
METHODS:
Focus groups and in-depth personal interviews were conducted in three regions of the Russian Federation with the population over 18 years of age (N=150) and with healthcare workers (N=150). The World Health Organization’s approach “Tailoring immunization programmes” was used. The purposeful sampling method was applied to select the study participants. Both individual and contextual factors influencing vaccination behavior were studied. The data were analyzed using content analysis, both among adult population and healthcare workers in primary healthcare settings.
RESULTS:
In population, the factors hindering the vaccination were insufficient availability of information, organizational difficulties (queues, uncomfortable appointment time for working people), and deficiencies in explanations and arguments from healthcare workers. The obstacles for healthcare workers were the lack of accessible and regularly updated information on vaccines, statistical information on morbidity, outbreaks, information materials for patients, as well as lack of communication skills and time for explanations during an appointment, and inaccessibility of patients who do not visit healthcare settings regularly.
CONCLUSIONS:
The study showed that most of the factors hindering vaccination are based on deficiencies in communication on vaccination issues, both at the systemic level "healthcare-population" and at the individual level "doctor-patient". Based on the obtained results, an educational video course for healthcare workers was developed, whish is intended to improve their communication skills. Also, a system for informing and reminding adult population about vaccination was developed, designed in accordance with the National immunization calendar and intended to improve their vaccine confidence.
