Birth weight and diabetes risk in adulthood: Evidence from the 1970 British Cohort Study

OBJECTIVES:
Low birth weight has long been hypothesized to predispose individuals to metabolic disorders in later life through developmental programming mechanisms. However, longitudinal evidence spanning the life course remains limited. This study examined the association between birth weight and diabetes risk in adulthood using data from the 1970 British Cohort Study (BCS70).
METHODS:
Data were drawn from eight sweeps of the BCS70, a nationally representative cohort of 17,196 individuals born in the UK in April 1970. Birth weight, gestational age, and maternal characteristics were recorded at birth. Adult parameters—including BMI, lifestyle behaviours, and self-reported diabetes diagnosis—were collected up to age 50. Logistic regression models estimated the odds of diabetes across birth weight categories (<2500 g, 2500–3999 g, ≥4000 g), adjusting for sex, gestational age, BMI, maternal smoking, and anemia. Results and analysis: Among 4,029 adults with complete data, diabetes prevalence was higher among those born with low birth weight (8.7%) than normal or high birth weight. After adjustment, adults with low birth weight had over threefold higher odds of diabetes compared with those ≥4000 g (OR = 3.13, 95% CI: 1.12–8.73, p = 0.03). Normal-weight infants also showed elevated odds (OR = 2.19, 95% CI: 1.01–4.77, p = 0.04). Lower adult BMI and moderate alcohol consumption were inversely associated with diabetes.
CONCLUSIONS:
Lower birth weight independently predicts higher diabetes risk in midlife, supporting the developmental origins hypothesis of chronic disease. Public Health Impact: These findings underscore the importance of maternal and early-life health in chronic disease prevention. Public health policies promoting optimal maternal nutrition and monitoring of low-birth-weight infants may help reduce the long-term burden of diabetes across populations.
