Developing and applying a logic model to evaluate health projects within Brazil’s Unified Health System: a case study

ABSTRACT
INTRODUCTION:
The Programme for Institutional Development Support of Brazil’s Unified Health System (PROADI-SUS) involves seven hospitals of excellence in partnership with the Brazilian Ministry of Health and implements triennial public health projects nationwide1. This study aims to describe the process of developing and applying a logic model conducted by the Hospital Sírio-Libanês (HSL) to support the design, monitoring, and evaluation of public health projects.

METHODS:
Logic Model and Theory of Change approaches were jointly applied with project teams through a structured process comprising2: (i) synchronous meetings to collaboratively develop logic matrices and indicators; (ii) systematic construction of logic matrices by breaking down the overall objective into specific objectives, followed by actions and deliverables, and concluding with expected results for each specific objective, while results and impacts reflect changes achieved over time through attainment of the overall objective; and (iii) development of indicator matrices linking indicators to expected results and deliverables, acknowledging the limitations of conducting comprehensive impact analyses within a three-year cycle.

RESULTS:
A total of 23 logic and indicator matrices were developed, covering 100% of PROADI-SUS/HSL projects. These matrices enable systematic monitoring of project results throughout the 2024–2026 triennium, with a total budget of approximately USD 124 million. For evaluation purposes, 43 studies are planned, including 30 outcome evaluations, 5 experimental studies, 4 economic evaluations, 2 implementation studies, and 2 impact evaluations. The main methodological approaches include cohort studies, randomised clinical trials, case reports, surveys, budget impact analyses, and qualitative interviews. Final and partial evaluation results are expected in 2026.

CONCLUSIONS:
The application of logic matrices strengthens the evidence base of PROADI-SUS projects and supports the consolidation of public health policies. The logic model is a valuable tool for identifying transformations promoted within the Healthcare Network by linking expected results to measurable indicators, thereby promoting accountability in public–private partnerships.
