Development and Validation of an Evaluation Model for Digital Health Interventions in Primary Health Care in Brazil

ABSTRACT
BACKGROUND:
The integration of digital technologies in primary health care requires robust monitoring and evaluation frameworks to ensure quality healthcare delivery aligned with sustainable development goals.1 Despite growing implementation of digital health interventions,2,3 there remains a critical gap in standardized evaluation models, particularly those assessing impacts on core primary care attributes: first contact, longitudinality, comprehensiveness, and care coordination.4 This study addresses this gap by developing and validating a comprehensive evaluation model for the Brazilian context.

METHODS:
An exploratory methodological study with qualitative approach was conducted between June 2021 and March 2024.5,6 The development process comprised four stages: literature review of existing models, initial model construction, expert validation through nominal group consensus technique, and model refinement.7 Seven experts participated through synchronous and asynchronous remote procedures, including electronic questionnaires with 18 subjective questions, videoconference workshops for collaborative discussion, and iterative review cycles using online collaborative documents until consensus was achieved.8

RESULTS:
The validated model integrates Donabedian's triad (structure, process, and outcomes) with primary care quality attributes.9 Structure encompasses four indicators: human resources, financial resources, infrastructure, and regulatory/strategic resources. Process is organized across technical, organizational, and relational dimensions. Outcomes are evaluated through products, intermediate results, and short/medium/long-term impacts, measured against seven quality pillars: efficacy, effectiveness, efficiency, optimization, acceptability, legitimacy, and equity.10 The model's distinctive feature is its explicit incorporation of primary care quality attributes as evaluation criteria.

CONCLUSIONS:
The QualiAPS Digital-Brasil model provides a contextually adapted framework for evaluating digital health interventions in primary health care, addressing the absence of standardized evaluation criteria in Brazil's digital health strategy.11,12 Unlike international models, it specifically measures impacts on essential primary care attributes.13 The model is currently being applied in Brazilian case studies and offers potential for adaptation in other countries and contexts, contributing to evidence-based implementation of digital health technologies in primary care settings worldwide.
