Development of an mHealth Exercise Application for Preventing Secondary Disability and Improving Quality of Life among Lower Limb Amputees

Individuals with lower limb amputation are a vulnerable group with a high risk of developing secondary disabilities, including reduced mobility, losing function, and having a lower quality of life. To address these risks in a resource-limited context, this mHealth study was conducted in Indonesia, where access to disability-inclusive preventive health services remains limited. In this setting, mHealth-based interventions offer an inclusive and scalable way to deliver promotive and preventive health programs for people with disabilities. A community based quasi-experimental study was conducted among individuals with lower limb amputation using a disability inclusive mHealth exercise application. Out of 25 participants enrolled, 22 successfully completed the 12-week intervention. The program comprised supervised community-based exercise sessions twice weekly and one home-based session weekly. The quality of life was assessed prior to and during the intervention utilizing the WHOQOL-BREF instrument (0-100 scale). Paired statistical tests were applied to evaluate changes in quality of life, and effect sizes were calculated using Cohen’s d. The mean overall quality of life score increased significantly from (56.70±15.99) at baseline to (66.62± 1.65) after the intervention, showing an average improvement of 9.93 points (p = 0.001, Cohen’s d = 0.79). Moreover, significant improvements were observed also in the psychological domain, where scores increased from (55.49±5.40) to (65.54±12.09), with an average increase of 10.04 points (p = 0.008, d = 0.62). Similar upward trends were observed also in the social relationships, environmental, and physical health domains, suggesting that the mHealth intervention may improve multiple dimensions of quality of life. The disability-inclusive mHealth exercise intervention significantly enhanced the overall quality of life for lower limb amputees, particularly in psychological well-being and social interactions. These findings endorse the implementation of adaptive, technology-driven exercise programs as an effective preventive public health strategy to avert secondary disabilities, diminish health disparities, and enhance community-based.
