ABSTRACT
INTRODUCTION:
Leprosy represents a major challenge for managers and health professionals. Epidemiological surveillance involves various actions for monitoring, early diagnosis, control, and prevention. This study aims to analyze the surveillance actions currently being developed in the State of Bahia.

METHODS:
This is a cross-sectional observational study. A qualitative-quantitative questionnaire was developed regarding assistance characterization, diagnostic and examination networks, treatment, and surveillance actions. The questionnaire was sent to regional health bases and forwarded to the epidemiological surveillance of each municipality. Data were analyzed using Microsoft Excel and SPSS v.26.

RESULTS:
The state has been developing leprosy surveillance actions in partnership with regional health bases, but the need to strengthen these actions to control the epidemiological situation in Bahia is evident. To ensure data quality and strategic management, it is essential to establish and standardize clear flows for case investigation, as well as patient monitoring and the use of follow-up instruments.

CONCLUSIONS:
Epidemiological surveillance actions are intrinsic to disease control and indispensable for monitoring, integral follow-up, and detailed investigation of all cases. The State Leprosy Program faces the challenge of strengthening actions across health macro-regions to reach the targets set by the Ministry of Health and the World Health Organization.
