ABSTRACT
BACKGROUND:
Through Central Chronic Medicine Dispensing and Distribution (CCMDD), Government seeks to widen access to chronic medicines, ease congestion at health facilities and maintain continuity of care. In rural communities such as Lusikisiki factors such as long distances to travel, poor transport system and poverty, low health literacy give rise to poor adherence and discontinuation of treatment. CCMDD has been widely rolled out but its impact on adherence to medication and disease control in rural Eastern Cape have not been adequately evaluated.

METHODS:
A retrospective quantitative study will be performed in CCMDD-registered patients from January 2024 to December 2025, using a stratified random sample. Two strata will be recruited: clinic-based pick-up points and external/private pick-up points. Data will be obtained from electronic medical records, CCMDD collection data and routine district performance reports. Pilot testing, written procedures, and consistent data collection techniques will be used to establish and maintain the validity and reliability of the instruments.

RESULTS:
The study is designed to offer evidence-based efficacy of CCMDD on access, adherence and clinical outcomes for patients in rural Lusikisiki. It will also document barriers to programme performance such as problems with the supply chain, access not just geographically but in terms of socioeconomics and outcomes relative to clinics or external pickup.

CONCLUSIONS:
The results of this study will generate evidence to the effectiveness of CCMDD programme in improving disease prevention and control amongst patients with chronic diseases. Through the analysis of adherence profiles and clinical measures (viral suppression, blood pressure and glucose control), the results will guide interventions aimed at increasing programme effectiveness, minimising complications. This note will add to knowledge about the effect of CCMDD as a public health intervention in the management of chronic diseases and inform evidence-based scale-up at district and national level.
