Evaluation of the effect of the national oral health policy on the provision of dental prostheses in primary care in the state of Bahia, Brazil

ABSTRACT
BACKGROUND:
The National Oral Health Policy (PNSB) enabled the incorporation of prosthetic rehabilitation into Primary Care through the implementation of the Regional Dental Prosthesis Laboratories (LRPD). This study aimed to evaluate the effect of the PNSB on the provision of dental prostheses in Primary Care in the state of Bahia, considering the different types of LRPD accreditation between 2008 and 2019.
METHODS:
The study was conducted in two stages. First, a descriptive ecological study was carried out with an analysis of outpatient production of dental prostheses according to the quantity produced and the temporal evolution of municipalities with accredited LRPDs throughout the study period. In the second stage, the effect of the PNSB on the provision of dental prostheses was analyzed through a linear regression model.
RESULTS:
The results showed greater production of maxillary and mandibular prostheses, both complete and partial, while the production of crown and temporary prostheses showed non-significant values. The time-trend analysis indicated an increase in the percentage of municipalities with accredited LRPDs between 2010 and 2011, followed by a reduction between 2012 and 2013. From 2014 onward, a new growth trend was observed, with 2019 being the year with the highest number of accreditations. An effect of the PNSB on prosthesis production was observed in municipalities in the state of Bahia with LRPDs accredited by the Ministry of Health (MS), with higher statistical significance, as well as in those accredited by both the MS and the Expansion Plan list. Conversely, a low-magnitude effect was observed in municipalities with accreditation exclusively through the Expansion Plan.
CONCLUSIONS:
The PNSB contributed to the expansion of prosthetic rehabilitation services in Primary Care, especially through the accreditation of LRPDs based on MS ordinances.
