ABSTRACT
INTRODUCTION:
Vaccination is one of the most effective public health interventions. Despite its proven benefits, it faces various controversies, particularly parental reluctance that persists and intensifies, leading to decreased herd immunity among children. This study aims to identify factors associated with parental reluctance to vaccinate children aged 0 to 24 months in the KTL health zone of Couffo department in 2025.

METHODS:
This was a cross-sectional, analytical, mixed-methods study conducted in the KTL health zone. A probabilistic method with multi-stage random sampling technique was used to select primary targets. Data were collected using a digitalized questionnaire via KoBoCollect. Analysis was performed using STATA 15 software, and factors associated with parental vaccine hesitancy were identified through multiple logistic regression with a significance threshold of 5% and a 95% confidence interval.

RESULTS:
A total of 494 households were surveyed. The mean age of household heads was 36.98 years. The proportion of vaccine hesitancy was 46%. Female gender (p=0.034), monogamous household (p=0.00), farmer occupation (p=0.00) and trader occupation (p=0.01), perception of vaccines as dangerous (p=0.00), side effects concerns (p=0.00), vaccine alternatives (p=0.026), geographical accessibility (p=0.00), and communication quality (p=0.00) were significantly associated with parental vaccine hesitancy in the KTL health zone.

CONCLUSIONS:
This study conducted in the KTL health zone revealed that parental vaccine hesitancy is linked to various social, perceptual, and structural factors, requiring targeted multisectoral responses to strengthen vaccine adherence.
