INTRODUCTION:
Reliable use of data is central to achieving equitable family planning access and accountability. The community oriented DMPA SC self injection acceleration in nigeria (CODSAiN) project demonstrated how community perspectives can be combined with logistics and health management systems to inform planning and delivery. Health educators coordinated integrated outreach, facilitated community dialogues, and ensured that evidence reflected both facility records and lived realities. This approach transformed routine data into actionable insights that guided inclusive strategies for family planning scale up.

METHODS:
Health educators across local government areas in the seven states coordinated dialogues with communities, supported community resource persons, and led integrated outreach. Routine data were triangulated with qualitative insights to inform demand generation and supply planning. supportive supervision was conducted to ensure quality assurance and adaptability to local contexts.

RESULTS:
Between january 2024 and july 2025, monthly self injection clients increased by 177% (24858 to 68926). uptake rose from 19% to 38%. Improved forecasting and distribution reduced stockouts, while facility and community collaboration enhanced trust and reliability. The involvement of health educators transformed data into practical tools that directly influenced program performance, equity, and access.

CONCLUSIONS:
CODSAiN demonstrates that data become impactful when participatory and inclusive. by positioning health educators as frontline facilitators of integrated outreach, family planning scale up was grounded in evidence while improving accountability, trust, and equity. This model highlights the role of health educators in bridging communities and health systems, offering replicable lessons for other low and middle income countries on strengthening health systems through demand generation, community engagement, and evidence driven programming.
