Human papillomavirus vaccination coverage in a state in Northeastern Brazil: temporal analysis of macro-regions in the years 2022-2024

ABSTRACT
BACKGROUND:
Analyzing the types of Sexually Transmitted Infections (STIs), HPV infection is one of the most common infections in the world1. Therefore, effective vaccination coverage is the main strategy for prevention in the population2.The aim of this study is to analyze HPV vaccination coverage over time in municipalities from a Brazilian state.

METHODS:
This ecological study included a temporal analysis3 from 2022 to 2024, based on data from age cohorts for boys and girls aged 9 to 14 years in the observation unit of the macro-regions of the state of Ceará, Northeast Brazil. Data was extracted through IntegraSUS, a public management transparency platform that included all official data from the Ceará State Health Department. The survey population considers a total of 1,802,435 children and adolescents aged 9 to 14. Absolute and relative frequencies were calculated. T-test was used to compare the mean of vaccination coverage in the population of each municipality between the sexes. The Jamovi 2.6.2.5 was used, and a p-value<0.05 was adopted.

RESULTS:
A total of 4,899 children and adolescents were vaccinated and registered on the platform: 1,442 (29.44%) in 2022, 1,697 (34.64%) in 2023, and 1,760 (35.93%) in 2024. The mean vaccination coverage rate was 0.739 (standard deviation (SD)= 0.318); this is lower than the national target (>0.90). The difference between boys and girls (0.605; SD=0.218; vs. 0.851 SD=0.239, p =< .001) was statistically significant.

CONCLUSIONS:
Despite a large amount of vaccines in the state of Ceará, there is a noticeable lack of HPV vaccination coverage. Public campaigns and interventions are essential to increase awareness, reduce access barriers, and mitigate inequities in vaccination coverage, even in countries with universal health systems.
