Impact of IFN-γ Secretion and IFN-γ +874T/A Genetic Polymorphism on Hepatitis B virus and Toxoplasma gondii Co-Infected Type-1 Diabetes Patients

Toxoplasma gondii infection is a widespread parasitic disease with the potential to cause severe health complications in immunocompromised individuals like Type-1 Diabetes Mellitus (T1DM) patient. Since, T1DM patients have suppressed immunity they are vulnerable for coinfection with hepatitis B virus and T. gondii. However, the association between HBV-T. gondii coinfection in T1DM is still unclear. Here, one hundred T1DM patients and equal number of healthy subjects were enrolled to investigate T. gondii and HBV prevalence in T1DM patients of Bangladesh. Both infections were confirmed by the presence of specific IgG antibodies in their serum using ELISA. The findings revealed that 25 % of T1DM patients exhibited positive for T. gondii chronic infection while 5% of them were coinfected with HBV. However, hematological analyses showed lower total WBC counts (P=0.008), circulating eosinophils (P=0.023), and neutrophils (P=0.045) in T1DM patients compared to healthy controls. Conversely, platelet counts (P=<0.0001) and monocytes (P=0.0032) were elevated in the T1DM group. Our data also showed that the HBV-T. gondii coinfected T1DM patients exhibited a significant increase in IFN-γ levels (P<0.0001) compared to both the T. gondii negative T1DM patients and healthy control. Furthermore, host IFN-γ +874T/A (rs2430561) gene polymorphism was investigated using ARMS-PCR to determine the association of HBV-T. gondii coinfection with T1DM. Our data demonstrated marked differences in A and T allele distributions between T1DM patients with T. gondii infection and the control group. Notably, among infected T1DM patients, the A/A genotype was more prevalent (50.50%) compared to the control group (3.16%), while the A/T genotype was less common (34.34%) among infected T1DM patients compared to the control group (97.84%). This suggests that homozygosity for the A allele may increase susceptibility to HBV-T. gondii coinfection. Further research is needed to determine the nationwide prevalence of HBV-T. gondii coinfection in Bangladesh.
