ABSTRACT:
Primary Health Care (PHC) is the structural axis of the Unified Health System (SUS) network, requiring constant qualification of clinical and managerial practices. In this context, the implementation of evidence-based clinical protocols represents a strategy to strengthen care practices. The Practical Approach to Care Kit (PACK) was developed in South Africa to support health professionals in low- and middle-income countries in making standardized, safe, and resolutive clinical decisions. In Brazil, its implementation began in 2015 in Florianópolis (SC), showing positive results in improving the work processes of PHC teams and encouraging its dissemination. This is an experience report developed during the training process, using participant observation, local management records, and field notes from workshops and training sessions In Porto Alegre, the Municipal Health Department began implementing PACK in the second half of 2025, with the training of municipal leaders, local multipliers, and subsequently, the qualification of teams from 134 health units in the city.. The stages included the training of 25 municipal leaders, 147 local trainers (physicians and nurses), and the initiation of workshops in the units, discussing clinical cases based on PACK flowcharts. The tool was well accepted, especially for its practicality and integration across different clinical areas. Challenges included the need for adaptation to local contexts, workload, and adjustment of care flows. Initial assessments indicate PACK’s potential to enhance standardization of conduct and patient safety. The implementation of PACK in Porto Alegre represented progress in consolidating evidence-based clinical practices in PHC, promoting professional qualification and improving problem-solving capacity. Continued training and result monitoring are essential to ensure the sustainability of this initiative.
