Implementing the Respiratory Syncytial Virus (RSV) Vaccination for Adults: A Global Perspective with Lessons from China

ABSTRACT
BACKGROUND:
Respiratory Syncytial Virus (RSV) is a growing public health concern among older adults, leading to severe respiratory illness, hospitalisation, and rising healthcare costs. To address this challenge, the World Federation of Public Health Associations (WFPHA), through its Immunisation Taskforce and in collaboration with European experts, has developed recommendations for the design and implementation of adult RSV vaccination programs.
METHODS:
A structured consensus-building process with international public health experts identified priority areas for action. The task force emphasised evidence-based strategies and practical approaches that can be adapted across countries, with particular attention to the opportunities and challenges facing China.
RESULTS:
Three priority areas were highlighted: (1) strengthening monitoring and data collection systems; (2) implementing effective vaccination strategies; and (3) enhancing training and public awareness. Building on these lessons, a case study from Shanghai demonstrates how a large urban centre can serve as a model for national rollout. Shanghai’s strong public health infrastructure, digital health integration, and experience with vaccination campaigns provide valuable insights into how China can lead in introducing adult RSV vaccination.
CONCLUSIONS:
China is uniquely positioned to advance adult RSV vaccination as part of its healthy ageing agenda. Lessons from the WFPHA Immunisation Taskforce, combined with Shanghai’s experience, can guide a comprehensive implementation strategy that informs national policy. Global dialogue among policymakers, experts, and practitioners will help translate these recommendations into sustainable immunisation programs. In the face of demographic change, protecting older adults through preventive measures such as RSV vaccination is vital to reducing disease burden, safeguarding well-being, and ensuring resilient healthcare systems.
