The classical epidemiological triangle (host, agent, and environment) has long guided the understanding of disease causation and informed public health interventions. However, it insufficiently captures the influence of community engagement (CE) on disease dynamics and intervention efficacy. This conceptual gap perpetuates an overly individual-centric approach, neglecting the sociocultural, behavioral, and structural determinants that operate at the community level. This paper proposes an expanded epidemiological framework that systematically integrates CE, positioning communities as active agents in shaping health behaviors, facilitating intervention uptake, and enhancing population-level outcomes. Drawing on evidence from epidemiology, implementation science, and community-based health initiatives, the framework elucidates the mechanisms through which CE modulates host susceptibility, agent exposure, and environmental determinants of disease. Integrating CE into epidemiological models enhances the design, targeting, and sustainability of disease prevention strategies, fosters equity, and promotes social accountability. This re-conceptualization represents a paradigmatic shift, bridging individual-level determinants with community-level processes, and offers actionable guidance for contextually responsive public health interventions.
