Integrating family planning and integrated service delivery: a community based model for rmncah+n services in nigeria

ABSTRACT
INTRODUCTION:
Missed opportunities for integration often result in fragmented care and inequities in nigeria. The community oriented DMPA SC self injection acceleration in nigeria (CODSAiN) project leveraged state led integrated service delivery platforms to deliver family planning information and services, ensuring that mothers accessing child immunization could also receive contraception. This approach optimized health worker time, reduced travel burden, and normalized family planning/child birth spacing as part of maternal and child health interactions, particularly in underserved areas with limited facility access.
METHODS:
Family planning counseling was systematically integrated into integrated outreaches across seven states. Health educators and community resource persons provided household counseling, while frontline providers linked family planning with child immunization visits and other activities. Demand generation was reinforced through local radio messaging, community dialogues, and endorsement from trusted leaders to foster acceptance and counter misinformation.
RESULTS:
Integration led to measurable improvements. contraceptive adoption increased by 177% between january 2024 and july 2025 (24858 to 68926). Women reported reduced travel and appreciation for comprehensive care. Health workers gained confidence in providing integrated services, while communities, including male partners and leaders, showed greater acceptance of family planning. The model improved efficiency, strengthened continuity of care, and enhanced the link between child health and maternal reproductive autonomy.
CONCLUSIONS:
Integrating family planning with immunization proved feasible, acceptable, and impactful in improving access and equity. This community based model reduced fragmentation, improved efficiency, and enhanced autonomy by embedding family planning within maternal and child health platforms. CODSAiN demonstrates how leveraging immunization programs can advance integrated rmncah+n services and strengthen public health systems in resource limited contexts.
