ABSTRACT
INTRODUCTION:
About 15% which is approximately 20 million women suffer complication during pregnancy annually. Every day estimated 800 women die from preventable causes related to pregnancy and childbirth. An average global rate of 18 deaths per 1000 live births is the highest risk of dying faced by children in their first month of life annually. Mining environmental contamination poses risks to maternal and neonatal outcomes in various ways such as increased risk of exposure to toxic chemicals, air pollution, water contamination, food contamination and stress and anxiety. These significantly increases the risk of adverse pregnancy outcomes and adverse neonatal birth outcomes. The study aimed at determining the impact of mining environmental contamination on maternal complications and adverse neonatal birth outcomes in Zambia.

METHODS:
A cross-sectional study design was conducted with the sample size of 337 participants. Data was collected from women attending postnatal care at selected facilities in Kitwe and Kapri Mposhi districts, with supplementary data collected from Kitwe Teaching Hospital and Kapiri General Hospital.

RESULTS:
It was found that about 22.7% of women attending maternity services at Kitwe Teaching hospital had at least a pregnancy related complication, whereas in Kapiri Mposhi about 8.6% of women attending maternity services had at least one complication. Adverse neonatal birth outcomes were recorded at 29.3% at Kitwe Teaching Hospital and 17.1% were recorded at Kapiri Mposhi General Hospital. Stillbirth (AOR (95%) [1.808 (1.115, 2.932)], gestational hypertension (AOR (95%) [2.292(1.422, 3.693)], and unsuccessful pregnancy (AOR (95%) [2.050 (1.716, 2.932)] were statistically associated with mining activities. In conclusion, mining activities may negatively affect both maternal health and birth outcome.
