ABSTRACT
INTRODUCTION:
osteoporosis is a systemic skeletal disorder characterized by low bone mass and microarchitectural deterioration, predisposing individuals to fractures. women, particularly postmenopausal, are disproportionately affected due to estrogen deficiency. although dual-energy x-ray absorptiometry (dxa) remains the diagnostic gold standard, its high cost limits accessibility in low-resource settings. dental panoramic radiography (dpr), a widely available imaging modality, may offer a low-cost alternative for bone mineral density (bmd) assessment in such contexts. this study aimed to assess mandibular bmd in cameroonian women using panoramic radiomorphometric indices.
METHODS:
an analytical cross-sectional study was conducted from november 2021 to june 2024 at the university of yaoundé i. a total of 188 panoramic radiographs of women aged 25–86 years were analyzed, provided that both mental foramina and mandibular cortices were visible. radiomorphometric indices mandibular cortical index (mci), gonial index (gi), antegonial index (ai), mental index (mi/mandibular cortical width), and panoramic mandibular index (pmi) were measured using smartdent software. data were analyzed using spss version 27.0 with a significance level of p<0.05.
RESULTS:
participants had a mean age of 43.1 ± 15.2 years, with 68.6% being premenopausal. mci classes i and ii predominated in both groups, while class iii was rare (<2%). there were no significant differences in gi, ai, mi/mcw, or pmi between pre- and postmenopausal women (p>0.05). age correlated negatively with left gi (p=0.023), left ai (p=0.039), and right mi/mcw (p=0.032), indicating age-related cortical thinning.
CONCLUSIONS:
panoramic radiographic indices provide a reliable, low-cost method for evaluating mandibular bmd in cameroonian women. despite its two-dimensional limitations, panoramic radiography shows potential as a screening tool for osteoporosis in resource-limited settings. further validation against dxa is recommended to establish population-specific thresholds for bone health monitoring.
