ABSTRACT
INTRODUCTION:
Digital health solutions are essential to strengthen health information systems, improve care coordination, and support continuity of care in public health networks. The World Health Organization highlights the importance of user-centered digital tools for improving information governance and clinical decision-making in primary health care¹. However, electronic health records frequently present fragmented information, excessive navigation steps, and usability limitations, which can hinder clinical decision-making, particularly in longitudinal and shared care contexts². This study reports the development and implementation of a personalized clinical summary screen designed to provide a concise, structured, and user-centered overview of patients’ recent clinical information.

METHODS:
This project was developed in 2025 through a partnership between the Municipal Health Secretariat of São Paulo and Hospital Israelita Albert Einstein, Brazil. The initiative was implemented across 14 primary care units, 4 psychosocial care centers, 3 outpatient urgent care services, and 1 emergency care unit, collectively responsible for approximately 411000 inhabitants and an average of 173000 monthly visits. A participatory approach was adopted, involving multiprofessional teams from primary care, mental health, and oral health. Collaborative workshops defined essential clinical data for longitudinal care, organized into thematic sections including patient identification, risks and vulnerabilities, chronic conditions, therapeutic goals, recent encounters, referrals, and monitoring of priority examinations. The summary screen was configured within the electronic health record system and disseminated to care teams.

RESULTS:
Implementation led to faster access to key clinical information, reduced navigation time, and improved communication among professionals. Quality and patient safety teams reported improved reconstruction of care trajectories, enhanced analysis of adverse events, and better identification of contributing factors. The initiative received an institutional award recognizing its positive impact on patient and workforce experience.

CONCLUSIONS:
User-centered digital interfaces, even with low technological complexity, can significantly improve information governance, clinical decision-making, and continuity of care in public health systems¹,².
