ABSTRACT
BACKGROUND:
Phonetic modelling tools are sensitive and effective in investigating the relationship between speech perception and production1, 2, constituting non-invasive and low-cost methods capable of generating clinically relevant inferences. Aligned with guidelines for the care of people with hearing impairment within the framework of Brazilian public health policies3,4, this study was articulated with the Singular Therapeutic Project4, 5 (PTS) as a central care strategy. The research aims to consolidate an innovative methodological approach grounded in Clinical Phonetics to support individualised therapeutic planning for cochlear implant users within the Care Network for People with Disabilities of the Brazilian Unified Health Public System (RCPD-SUS).
METHODS:
A clinical-phonetic framework was developed based on perceptual-auditory, acoustic, and articulatory analyses of speech samples from 20 adults, with severe-profound sensorineural post lingual HL, were evaluated before and after cochlear implantation. Ethical approval was obtained from an independent Research Ethics Committee in Brazil (6.759.381). Individuals were instructed to read the Consensus Auditory-Perceptual Evaluation of Voice sentences6, in usual and comfortable voice. Perceptual-auditory evaluation was based in qualitative procedures and vocal pattern categorisation using the Vocal Profile Analysis Scheme adapted to Brazilian Portuguese7. The acoustic signal was imported into Praat 6.4.08 for the extraction of objective parameters through acoustic analysis, such fundamental frequency, formants and vocal stability measures. Data were analysed longitudinally and integrated into an interdisciplinary perspective to support speech-language therapeutic decision-making within the PTS.
RESULTS:
Phonetic outcomes enabled the identification of changes in speech perception and production patterns following cochlear implantation, demonstrating sensitivity to auditory, phonatory, and articulatory adaptations over time. The integrated analysis supported the characterisation of individual therapeutic needs and functional outcomes, considering barriers and functionality.
CONCLUSIONS:
Models based on Clinical Phonetics constitute robust and accessible tools for the construction of evidence-based PTS, strengthening interdisciplinary care, prognostic monitoring, and inclusive practices within the RCPD-SUS.
