Resistance Pattern of candidiasis infection in patients attending the Buea Regional Hospital

ABSTRACT
BACKGROUND:
Candida species are typically human microbiota that can be found in the skin but become opportunistic when the immune system is compromised. The growing antifungal resistance to Candida has made managing candidiasis challenging, increasing morbidity and mortality rates.
OBJECTIVE:
This study aimed to investigate the resistance patterns of Candida species to antifungal agents in patients at the Buea Regional Hospital.
METHODS:
A hospital-based cross-sectional study was conducted at the Buea Regional Hospital. Various samples (wound, vaginal and the ear swaps, urine, faeces, urinary catheter) were collected and cultured using SDA. A gram stain and germ tube test were performed to identify Candida species and differentiate C. albicans from other candida species. Antifungal susceptibility testing was done using the disc diffusion method.
RESULTS:
The study involved 251 participants (78 males (31.1%), 173 females (68.9%)). The majority were between 21-30 years old (29.1%), followed by those aged 31-40 years (27.1%). Vaginal swabs (41.4%) and urine (25.9%) were the most common samples collected. Of the 251 samples, 111 (44.2%) showed positive growth for Candida, with 79 (31.5%) identified as C. albicans and 32 (12.7%) as non-C. albicans. The antifungal susceptibility test revealed varying levels of resistance: fluconazole (1.9%), miconazole (1.6%), econazole (7.1%), nystatin (5.6%), and amphotericin B (23.9%).
CONCLUSIONS:
This study highlights the prevalence of Candida species in clinical samples, with C. albicans being the most common isolate. The antifungal susceptibility test revealed varying levels of resistance, with Amphotericin B showing a concerningly high resistance rate. These findings emphasize the need for continued surveillance and rational use of antifungal agents.
