ABSTRACT
BACKGROUND:
Vaccination is one of the main public health strategies; however, vaccination coverage in Brazil has been declining over the past decade, a scenario worsened by the Covid-19 pandemic. In Bahia, factors such as vast territorial extension, socioeconomic inequalities, and operational challenges contribute to this situation.

OBJECTIVE:
To analyze the risk classification for the transmission of immunopreventable diseases in the state of Bahia in 2023.

METHODS:
This is an ecological study based on public-domain data available on the LocalizaSUS platform, the Brazilian Institute of Geography and Statistics (IBGE), and the Primary Care Monitoring and Evaluation Report (CAMAB) of Bahia. Municipalities were classified according to indicators of vaccination coverage, homogeneity of coverage, and population size. Epidemiological risk was categorized into five levels (very low to very high).

RESULTS:
Bahia did not meet the vaccination coverage targets of the basic childhood schedule nor the dTpa vaccine for adults and pregnant women, revealing heterogeneous performance across municipalities. Although most municipalities (61.6%) were classified as very low or low risk, they account for only 31.37% of the state population. In contrast, 40.54% of the population lives in 4% of municipalities classified as very high risk, all of which have large population sizes. An increase in population size and in the Municipal Human Development Index was associated with reduced homogeneity of vaccination coverage and increased epidemiological risk, indicating an inversion of the expected pattern of collective protection.

CONCLUSIONS:
The methodology adopted and the results obtained may assist public managers in formulating more effective local immunization strategies, contributing to reducing the risk of transmission of immunopreventable diseases. The findings reinforce the need for more targeted interventions in the post-pandemic period, aiming to restore vaccination coverage and strengthen the National Immunization Program.
