ABSTRACT
INTRODUCTION:
According to International Diabetes Federation, half a billion of world population are living with Diabetes Mellitus of which 90% has Type 2 Diabetes Mellitus.At current incidence rate, it is estimated that more than 700 million of population will be living with Type 2 Diabetes Mellitus by 2045 and 75% of this will be in developing countries [1]. Adherence to medication is a complex phenomenon and Kerala, a state in southern part of India has a prevalence of 74% non adherence [2] . There isn’t adequate national level data which shows prevalence of adherence . Hence it is important to understand different factors involved in adherence to Oral Hypoglycemic Agents and the facilitators & barriers [3].
METHODS:
Qualitative exploratory approach was used and study setting was Oachira panchayath, Kollam district. Adults with Type 2 Diabetes Mellitus on OHA since past 6 months were included . Purposive sampling was used and thematic analysis was done
RESULTS:
, 4 different themes were identified – narratives and insights into adherence, uncovering the enablers, breaking through the impediments and dissecting the catalysts. There was a general perception regarding “ tablets” prescribed for illnesses and OHA was considered as another “ tablet” . There was a tendency to normalize Type 2 Diabetes Mellitus leading to not considering taking OHA as relevant or a necessity . Fear regarding complications was motivating factor to continue adherence . Presence of co morbidities and stress associated with job and household responsibilities was a de motivating factor.
CONCLUSIONS:
Adherence to Oral Hypoglycemic Agents in Type 2 Diabetes Mellitus is a complex phenomenon which is influenced by perceptions of patient , psychosocial factors and co morbidities. Strengthening primary health care, context specific and patient centric approach is the need of hour to improve adherence and long term outcomes.
