Social determinants of childhood tuberculosis in the 100 million brazilian cohort

ABSTRACT
INTRODUCTION:
this study aimed to analyze the sociodemographic factors associated with childhood tuberculosis in the cohort of 100 million Brazilians in the period of 2008-2018.
METHODS:
a cohort study with children and under 15 years old from the 100 Million Brazilians cohort linked to tuberculosis cases in the same age group from the National Notifiable Diseases Information System. The study period was January 1, 2008 to December 31, 2018. Data analysis was performed using the Poisson model with robust variance.
RESULTS:
21,619,026 individuals and 5,743 tuberculosis cases were included. Childhood tuberculosis was associated with living in urban area (IRR 1.61, CI95% 1.48-1.74), and it was higher in the north (IRR 1.69, CI95% 1.51-1.89) and southeast (IRR 1.50, CI95%1.32-1.70) regions. A higher risk of TB included indigenous (IRR 12.39, CI95% 10.60-14.47) or black (IRR 2.16, CI95%1.90-2.46) race/ethnicity compared with white race/ethnicity. Higher income was associated with a lower risk of TB.
CONCLUSIONS:
this study contributes to the understanding of the epidemiological situation of childhood tuberculosis over the past decade in Brazil. This study enhances the understanding of childhood tuberculosis in Brazil and emphasizes the importance of implementing public health policies aimed at addressing structural inequalities, reducing the disproportionate burden among Indigenous and Black children, and tackling socioeconomic disparities that exacerbate vulnerability in high-risk regions.
