Strategy.AI Thon: Conceptualizing Innovation and Strategic Governance Driven by AI through a Hackathon in Brazil

METHODS:
Addressing the challenges of an aging population in developing nations requires more than just technology; it requires rapid, structured strategic planning. This paper presents the development process of "GenerAtivas," a comprehensive strategic proposal designed during the Estratég.IA Thon—a pioneering strategy-focused hackathon for the public sector. Unlike traditional coding marathons, this challenge focused on the "Strategy-as-Practice" approach, utilizing agile tools like the X-Matrix (Hoshin Kanri) and Project Canvas to conceptualize how Artificial Intelligence could be integrated into the Brazilian Unified Health System (SUS). Project Potential and Innovation: The proposed "GenerAtivas" strategy outlines a digital ecosystem aimed at enhancing the National Policy for the Elderly (PNPI). Although currently a conceptual model rather than a deployed solution, its design offers significant potential for the Digital Health axis. The framework proposes an AI platform integrated with the National Health Data Network (RNDS) to support clinical decision-making, specifically for early cognitive decline screening. The experience of the challenge allowed the team to map complex governance structures involving federal and municipal levels, ensuring that the AI’s output would ethically and technically surpass current analog processes. Discussion on Digital Governance: A key outcome of this experience was the robust risk-mitigation framework developed. The proposal identifies "prohibited risks," such as algorithmic bias in age-related care, and suggests digital literacy programs as a prerequisite for technological adoption. This strategic foresight is crucial for ensuring that AI acts as a tool for equity rather than exclusion.
CONCLUSIONS:
The "GenerAtivas" case illustrates that intensive, collaborative challenges like the Estratég.IA Thon are essential for digital health transformation. They provide a safe environment to prototype complex policies, allowing public managers to refine feasibility and governance before large-scale investment. This conceptual model serves as a scalable roadmap for integrating AI into public health systems globally.
