INTRODUCTION:
Effective vaccine safety surveillance is vital for maintaining public confidence and ensuring global health security. In Nigeria, reports of adverse events following COVID-19 vaccination (AEFI) have raised concerns about vaccine hesitancy and reporting, efficiency, especially in resources-limited and border communities.

METHODS:
A cross-sectional study was conducted among 416 COVID-19 vaccine recipients using a structured questionnaire. Data were analyzed using SPSS v20. Descriptive and inferential statistics were used to assess relationships between AEFI experience, demographic variables and reporting behaviors.

RESULTS:
Of 396 valid responses, 89.9% reported at least one AEFI, most commonly muscle pain ( 29.8%), headache (15.4%) and fever (13.9%). About 71.5% reported AEFI to health facilities, while 31.8% did not report due to lack of knowledge, fear of blame or unavailability for forms. Significant associations were observed between reporting practice, education level and prior AEFI training (p<0.001).

CONCLUSIONS:
AEFI surveillance remains suboptimal, constrained by knowledge gaps and weak reporting systems. Strengthening surveillance training, ensuring reporting tools availability and integrating community feedback mechanisms are critical for sustaining vaccine confidence and preparedness for future outbreaks.
