Title: Turnover Intention Among Nurses in Public Hospitals in Jilin Province, China: Prevalence and Links With Regulatory Emotional Self-Efficacy, Compassion Fatigue, and Quality of Work Life

BACKGROUND:
Nurse turnover intention threatens staffing stability in public hospitals. This study assessed the level of turnover intention among nurses in Jilin Province, China, and examined its relationships with regulatory emotional self-efficacy, compassion fatigue, and quality of work life.

METHODS:
A multistage, face-to-face survey was conducted in November 2021 in 124 public hospitals in Jilin Province. Of 2,480 nurses approached, 2,412 valid questionnaires were analyzed (97.26%). Measures included turnover intention, regulatory emotional self-efficacy, compassion fatigue, and quality of work life, plus sociodemographic and work characteristics. Correlates of high turnover intention were examined using binary logistic regression. Pathways among key constructs were tested using partial least squares structural equation modeling (PLS-SEM).

RESULTS:
The mean turnover intention score was 1.91 ± 0.64; 1,630 nurses (67.85%) reported low turnover intention and 782 (32.42%) reported high turnover intention. In logistic regression, age, monthly income, average daily working hours, night shifts, regulatory emotional self-efficacy, compassion fatigue, and quality of work life were significant correlates of turnover intention (all P < 0.05; Cox and Snell R² = 0.15; Nagelkerke R² = 0.22). In PLS-SEM, regulatory emotional self-efficacy and quality of work life were negatively associated with turnover intention, whereas compassion fatigue was positively associated with turnover intention. Compassion fatigue and quality of work life partially mediated the association between regulatory emotional self-efficacy and turnover intention, and quality of work life partially mediated the association between compassion fatigue and turnover intention.

CONCLUSIONS:
Turnover intention among nurses in public hospitals in Jilin Province was substantial. Interventions that enhance regulatory emotional self-efficacy, reduce compassion fatigue, and improve quality of work life may help mitigate turnover intention.
