ABSTRACT
INTRODUCTION:
The Philippines is characterized by an aging population and a high prevalence of chronic non-communicable diseases, integrating digital health and artificial intelligence (AI) into long-term care. However, it still grapples with structural inequities, fragmented service delivery, and limited institutional capacity. This study aims to develop and validate a consensus-based framework for integrating digital health and AI into LTC in the Philippines, ensuring coherence with global standards, ethical principles, and the contextual realities of the universal healthcare system.
METHODS:
The research team co-developed initial thematic priorities and guiding statements during scholarly collaboration at the TMUN Advanced Healthcare Professional Training Program in Taipei (July 7–18, 2025). The iterative e-Delphi method was subsequently employed to achieve expert consensus. Nineteen conveners participated in the validation phases.
RESULTS:
Consensus was generated across five core themes. Interprofessional Collaboration (IPC) is essential for standardizing AI integration and ethical data use. Policy integration requires comprehensive policies for system-wide, cost-effective, and equitable access. The responsible and ethical use of AI mandates strengthened data privacy frameworks, Data Privacy Act compliance, ethical training, and algorithmic bias mitigation. Value-based Healthcare (VBHC) must prioritize continuity, coordinated care, and patient-centered outcomes. Finally, the integration of AI-assistive technology in education is necessary to foster shared competencies, enable adaptive learning, and enhance clinical readiness in LTC settings.
CONCLUSIONS:
Realizing the potential of digital health and AI in Philippine LTC requires systemic reforms, coordinated investment, policy alignment, and capacity building. Key strategies include embedding AI and digital health within the UHC framework, institutionalizing culturally responsive collaborative frameworks, mandating national interoperability standards, and ensuring sustained financing mechanisms, potentially through PhilHealth, to promote equity and sustainability. Continuous ethical education and stakeholder engagement are critical to ensuring AI systems enhance, rather than replace, human judgment and patient-centered care.
