ABSTRACT
INTRODUCTION:
Telemonitoring is an operational modality of telemedicine that allows the remote and continuous collection and transmission of vital and clinical parameters through sensors interacting with the patient. Telemonitoring is performed through dedicated devices and applications that collect data and transmit them to a platform via secure channels, enabling continuous updating of the patient’s clinical status. According to the literature, telemonitoring is particularly suitable for the evaluation of patients with respiratory failure. The hospital of cittadella, padua, italy, has a pneumology ward with 20 beds and 6 semi-intensive beds, and serves a population of 350000 inhabitants.

METHODS:
The hospital of cittadella implemented an integrated telemedicine model for the remote monitoring and management of patients affected by obstructive sleep apnea syndrome (OSAS) treated with continuous positive airway pressure (CPAP). The model includes the delivery and activation of CPAP devices equipped with data transmission modules by a respiratory physiopathology technician.

RESULTS:
During 2025, a total of 156 patients with OSAS were enrolled. For 60 of them, follow-up appointments have already been scheduled for the entire year 2026, with telemonitoring intervals of 3, 6, or 9 months according to clinical needs. All patients attended an initial in-person visit for training, CPAP device delivery, and signing of informed consent for personal data processing. The main benefits observed were: 1. improved disease control and treatment adherence; 2. therapy personalization; 3. reduction of routine outpatient visits; 4. possibility of monitoring and follow-up also in remote areas; 5. reduction of indirect costs (hospital visits, admissions, complications).

CONCLUSIONS:
This project highlights how telemedicine represents an effective tool in the management of patients with OSAS treated with CPAP. Telemonitoring allows improved control of therapeutic adherence and timely interventions in case of critical issues, while reducing the need for in-person outpatient visits.
