Climate emergencies and socio-environmental inequalities disproportionately affect the most vulnerable populations. Extreme events such as heat waves, cold waves, and floods directly impact health outcomes. Epidemiological studies and interdisciplinary analyses indicate that the interaction between social determinants and climate change amplifies health risks, requiring integrated responses in public policies and global health systems to promote resilience and equity.This retrospective study analyzed epidemiological notifications of emergency room visits from January 2024 to December 2025, based on secondary data. Causes associated with climatic events included leptospirosis, arboviral diseases, flu-like syndromes, and self-inflicted violence, reported by the Rio Operations Center (COR).During the study period, 132,833 emergency care visits were recorded. Twenty-seven cases of leptospirosis were identified, with a peak of 65.21% during months of highest rainfall, mainly in socially vulnerable neighborhoods. Thirty-four dengue cases were concentrated in Maré and Cacuia, with peaks in February and March. Fifty-four hospitalizations due to severe acute respiratory syndrome (SRAG) occurred predominantly among elderly residents, with a significant increase in June and July. A total of 285 cases related to self-inflicted violence were reported, mostly among women (72.1%) aged 20–39 years, followed by adolescents aged 13–19 years.The results demonstrate an association between intense rainfall and leptospirosis in vulnerable territories, while dengue incidence coincided with periods of extreme heat alerts. SRAG cases increased during months of pronounced climatic variability. Environmental racism amplifies health risks in marginalized areas, reinforcing inequalities in exposure and impact.The COR plays a critical role in environmental risk monitoring and prevention. Self-inflicted violence reflects social vulnerability and gaps in mental health support. Integrated public policies linking health, social justice, and socio-environmental development are essential, with intersectoral monitoring and response protocols prioritizing high-risk territories.
