The prevalence of periodontitis in adults with Type 2 Diabetes Mellitus: A Systematic Review

ABSTRACT
BACKGROUND:
The comorbid relationship of periodontitis and Type 2 Diabetes Mellitus (T2DM) underpins the importance of defining the burden of periodontitis in T2DM patients. The objective of this study was to conduct a systematic review to determine the prevalence of periodontitis in adults with T2DM.

METHODS:
The review was registered with Prospero (CRD42020219199) and conducted according to PRISMA guidelines. Study eligibility was determined by the clinical periodontal diagnostic methods to diagnose periodontitis; clinical attachment loss, probing depths, alveolar bone loss, bleeding on probing, as well as screening tools such as CPTIN, CPI or BPE. Eligibility for inclusion of the study was when T2DM was diagnosed using HbA1C. The following search engines were accessed; Pubmed, Science Direct, Ebscohost (academic search complete; dentistry and oral sciences), Scopus, Web of Science, Elicit and Google Scholar. The risk of bias of the included studies was assessed using the Joanna Briggs Institute critical appraisal tools. Prevalence of periodontitis was stratified geographically and over time using a random effects model.

RESULTS:
101 included studies yielded a sample size of 345311 T2DM adults. The pooled effect size of periodontitis prevalence in T2DM was found to be 60.3%. The highest prevalence was found in Indonesia (95%) and the lowest in Iran (14.5%). `There were negligible differences in prevalence of periodontitis reported over time in this population.

CONCLUSIONS:
The limitations of synthesized included studies included that not all studies stratified their results by sex, reported on related risk factors such as smoking and as with all periodontal epidemiological studies, there were variable means of case definitions and this was accepted as the authors had presented it to be. The high prevalence of periodontitis in T2DM emphasizes the importance of periodontal screening in T2DM. The study was self-funded.
