Tinnitus in Switzerland: National Prevalence, Age Gradients, and Cultural-Linguistic Differences within a Unified Health System

ABSTRACT
BACKGROUND:
Tinnitus is a prevalent non-communicable condition that can impair well-being, contribute to psychological distress, anxiety and sleep problems, and increase healthcare use. While cross-national studies show wide variation in tinnitus prevalence, meaningful heterogeneity within countries is rarely quantified. Switzerland offers a natural setting with three distinct linguistic and cultural regions (German-, French- and Italian-speaking) that share the same healthcare system, identical occupational health and safety regulations (including hearing-protection standards), and comparable access to care. This context enables the examination of regional differences in tinnitus burden while largely controlling for structural and regulatory factors.
METHODS:
We conducted a nationally representative, population-based survey using four independent online survey waves in 2025 (two in January and two in June). Adults aged 18–75 years were sampled, and post-stratification weights ensured national representativeness. Items assessed tinnitus frequency over the past year, tinnitus-related distress, and hearing difficulties. Analyses examined age gradients and regional differences across outcomes.
RESULTS:
Among 4,439 respondents, 50.2% reported at least occasional tinnitus, 17.2% frequent tinnitus, and 8.7% constant tinnitus. The risk of frequent and constant tinnitus increased with age, whereas occasional tinnitus decreased across older age groups, suggesting a shift from transient to persistent forms over the life course. Pronounced regional differences were observed: constant tinnitus was highest in the German-speaking region (9.4%) versus the French- (7.3%) and Italian-speaking regions (6.1%). Severe tinnitus-related distress was highest in the German-speaking region (10.9% of those with tinnitus), compared with 4.4% in the French- and 4.7% in the Italian-speaking regions.
CONCLUSIONS:
Tinnitus represents a substantial public-health burden in Switzerland, with pronounced age gradients and strong cultural-linguistic heterogeneity within a unified system. Cultural differences in symptom appraisal and reporting will be discussed in the presentation. These findings support targeted prevention, hearing conservation strategies, and region-sensitive service planning in ageing populations.
