ABSTRACT
BACKGROUND:
The Unifesp Digital Health Hub is a strategic initiative for the digital transformation of the Brazilian national health system, aiming to improve quality of care, expand access, and reduce health inequalities (1,2). The Hub targets areas requiring complex coordination, where digital solutions can strengthen care delivery, particularly for vulnerable populations such as people experiencing psychological distress, Indigenous peoples, rural and homeless populations, and individuals affected by violence (3–5). This study analyzes the implementation of the Digital Health Hub, highlighting initial results and key challenges.

METHODS:
An implementation study using mixed methods was conducted with quantitative and qualitative data produced between November 2024 - January 2026. Data sources included institutional documents, technical reports, and meeting records. Qualitative data were analyzed through thematic analysis to identify central implementation aspects. Quantitative data described the scope and progress of activities during the first year of implementation.

RESULTS:
Priority areas were defined based on academic expertise and health system needs, including digital health skills development; maternal and child health; mental health; care for LGBTQIAPN+ populations; population health and assistive technologies; and selected medical specialties such as ophthalmology. The initiative engaged 87 health professionals to deliver tele-education, teleconsultation, teleinterconsultation, teleconsulting, telediagnosis, and teletriage services across nine municipalities. An associated educational program involved 134 scholarship holders, mainly students and health system professionals, contributing to the expansion and local integration of activities within health services. Key challenges included limitations in telehealth platform functionality, insufficient technological infrastructure, lack of interoperability between information systems, and the need for continuous professional training.

CONCLUSIONS:
The Digital Health Hub demonstrates the potential of digital health strategies to expand access and support care delivery in underserved settings. However, its consolidation as a structuring health policy requires addressing persistent regional inequalities in technological infrastructure, limited connectivity in vulnerable territories, and fragmented health information systems.
