Bacterial agents and their multidrug-resistance profile among patients suspected of bacterial infections in Hakim Gizaw Hospital, Debre Berhan, Ethiopia

Background and

AIM:
Nowadays, there are serious threats due to resistant bacteria globally. Resistant bacteria are a growing challenge in Ethiopia, and up-to-date data on it is urgently required for effective interventions. This study aimed to determine the occurrence of multidrug-resistant bacterial isolates among patients presented to Hakim Gizaw Hospital (HGH).

METHODS:
A health-facility-based cross-sectional study was conducted in HGH, Debre Berhan, Ethiopia. All patients presented to HGH and diagnosed with bacterial infections were eligible for the study. Depending on the clinical presentation of the patient, different clinical samples were investigated in microbiology laboratory following standard operating procedures. The analysis was carried out using SPSS version 27.

RESULTS:
A total of 252 study participants were included in the analysis. Gender distribution was almost equal for male (50.8%) and female (49.2%). Among 252 study participants, only 103 (103/252, 40.9%) were culture positive, regardless of positive for one or two bacterial isolates. Gram-positive bacteria (65/115, 56.5%) were the predominant compared to Gram-negative bacteria (50/115, 43.5%). S. aureus (46/115, 40%) was the predominant followed by E. coli (13/46, 11.3%). E. coli showed higher resistance to Ciprofloxacin (CIP), Doxycycline (DOX), Trimethoprim-sulfamethoxazole (SXT), Cefoxitin (FOX), and Ceftaroline (CTR) (all 61.5%). K. pneumonia showed resistance from 27.3%-90%, with low resistance against GM and high resistance against Ampicillin (AM, 90.9%) and Amoxicillin-clavulanate (AMC, 90.9%). S. aureus showed higher resistance against CIP (58.7%), SXT (60.9%), and Erythromycin (E, 67.4%). Multidrug-resistant (MDR) rate was 62.6% (72/115) in the present study. Among Gram-negative bacteria, the prevalence of ESBLs and carbapenemase production was 66% (33/50) and 40% (20/50), respectively.

CONCLUSIONS:
Many bacterial infections were linked with high MDR bacterial isolates, including ESBL and carbapenemase-producing Gram-negative bacteria in the study area. Thus, diagnosis and treatment of patients must be evidence-based, and need timely, tailored public health interventions.
