ABSTRACT
BACKGROUND:
Small Cell Lung Cancer (SCLC) is the most aggressive histological type of lung cancer, with poor survival. There are very few studies specifically addressing the prevalence of risk occupations and their distribution in this histological type. The aim of this study is to describe the frequency of risk occupations in this infrequent but lethal histological lung cancer type.

METHODS:
A cross-sectional nationwide study was conducted using data from the Thoracic Tumor Registry (TTR) of the Spanish Lung Cancer Group. This registry includes more than 80 hospitals throughout Spain. All SCLC cases diagnosed between 2016-2025 were included and all of them followed up until death or until the last follow up. A descriptive analysis of the sociodemographic, clinical and work-related variables was conducted globally and broken down by sex.

RESULTS:
The study included 4966 SCLC cases, 27% women. Median age at diagnosis was 67 for men and 63 for women. Only 1% and 5% of men and women were never-smokers, respectively. 25.2% of men had worked in risk occupations of lung cancer compared to 11.2% in women. The most frequent risk occupations were construction workers (9.3%) and professional drivers (3.5%) in men and professional cleaners (7.6%) and mechanics (0.7%) in women. Mean age at diagnosis was in all cases below 70 years old. Professional cleaners and hairdressers had a mean age at diagnosis of 61 and 64 years old, respectively.

CONCLUSIONS:
Lung cancer-related occupations are highly prevalent in SCLC cases in men (25%) and women (12%), apart from tobacco consumption. Interestingly, the frequence of certain occupations traditionally more present in women was quite high, such as professional cleaners and hairdressers. Mean age at diagnosis was much lower for certain risk occupations.
