Quantifying the incidence of lung cancer by histological subtypes in China, 2022

ABSTRACT
INTRODUCTION:
Lung cancer is the leading cause of cancer incidence and mortality in China. Yet, its distribution by histological subtypes remains unclear. Our study aims to quantify the incidence burden of four major subtypes and inform future prevention strategies.
METHODS:
We extracted data from 157 population-based cancer registries in China included in the Cancer Incidence in Five Continents (CI5) Volumes XII to calculate the proportions of major histological subtypes [squamous cell carcinoma (SCC), adenocarcinoma, small-cell carcinoma, large-cell carcinoma] of lung cancer. We multiplied the obtained proportions by sex and agegroup by the number of lung cancer cases by sex and agegroup in 2022 calculated based on 700 registries previously and calculated incidence rates for four histological subtypes. In addition, we also quantified the burden of lung cancer incidence attributable to smoking in 2022.
RESULTS:
In 2022, the estimation of new lung cancer cases was 658,722 and 401,862 among males and females in China, respectively. In males, 31.0% of lung cancer cases was SCC [age-standardized incidence rate (ASIR): 16.0/100,000], 45.6% was adenocarcinoma (ASIR: 24.0/100,000), 10.6% was small-cell carcinoma (ASIR: 5.5/100,000), and 7.6% was large-cell carcinoma (ASIR: 3.9/100,000). In females, the corresponding proportions were 11.8% (SCC; ASIR: 3.4/100,000), 72.6% (adenocarcinoma; ASIR: 22.3/100,000), 5.1% (small-cell carcinoma; ASIR: 1.5/100,000), and 6.9% (large-cell carcinoma; ASIR: 2.0/100,000). In addition, 54.9% (95% confidence interval[CI]: 47.6%-61.2%) of lung cancer cases among male individuals aged 20 years and over were attributable to smoking, corresponding to an ASIR of 48.06/100,000. The PAF among females (8.9%, 95%CI: 7.9%-9.9%) was significant lower than males, with an ASIR of 4.1/100,000.
CONCLUSIONS:
In China, incidence differs across the four lung cancer subtypes, with adenocarcinoma being the most common. Tailoring prevention and control strategies to specific subtypes is essential.
