Sociodemographic factors associated with knowledge and practices regarding the prevention of home accidents among parents and caregivers of children under five years of age, Cartagena, Colombia, 2025

ABSTRACT
BACKGROUND:
Home accidents are among the leading preventable causes of morbidity in children under five years of age and are closely related to the family environment and the social conditions of parents and caregivers. This makes it necessary to examine the influence of these factors on preventive knowledge and practices.

METHODS:
An observational, analytical, cross-sectional study was conducted. The sample consisted of 477 participants, selected through cluster sampling. Data were collected using a structured questionnaire based on the Colombian Ministry of Health guidelines for the prevention of home accidents in children, which was previously evaluated by experts and pilot tested. Statistical analysis included descriptive statistics and binary logistic regression to identify associations, performed using Stata software.

RESULTS:
Overall, 65.4% of participants demonstrated good knowledge regarding the prevention of childhood home accidents, while 34.5% showed insufficient knowledge. Regarding practices, 61.6% reported adequate practices and 38.3% reported inadequate practices. In the multivariable analysis of knowledge, significant associations were observed with higher educational level (OR = 8.88; 95% CI: 2.39–32.93; p = 0.001), income above one minimum wage (OR = 22.97; 95% CI: 6.54–80.63; p < 0.001), and extended family type (OR = 31.11; 95% CI: 12.61–76.77; p < 0.001). In the practices model, only income above one minimum wage (OR = 6.72; 95% CI: 1.70–26.54; p = 0.007) and having adequate knowledge (OR = 48.31; 95% CI: 20.99–111.23; p < 0.001) were significantly associated with adequate practices.

CONCLUSIONS:
A high proportion of adequate preventive knowledge and practices regarding home accidents was observed. Sociodemographic characteristics played a relevant role in the adoption of these behaviors, highlighting the need for educational and health promotion interventions tailored to the social context of parents and caregivers.
